Abstract:

It was analyzed the immediate and long-term results of surgical treatment of 48 patients with chronic atherosclerotic occlusion of the femoral-popliteal-tibial segment in the stage of critical ischemia. Patients underwent femoral popliteal bypass in the isolated segment by reversed great saphenous vein. Five-year shunt patency was 54.2%, the preservation of limbs - 64.6%. To predict the duration of shunt patency in the isolated segment of the popliteal artery developed ratio of outflow channel consistency was used. When the ratio was 0.8±0.18 average duration of shunt patency was 0.68±0.1 years. The coefficient of 1.22 ± 0.05 was associated with 2.33±0.58 years, value of 1.28±0.13 - with 3.80±1.30 years, value of 1.65±0.25 - with more than 5 years. It was concluded that in patients with atherosclerotic lesions of the femoral-popliteal-tibial segment and critical ischemia femoral-popliteal bypass in the isolated segment should be performed in the presence of a consistent outflow channel. Developed ratio of outflow channel consistency is advisable to use for prediction of shunt functioning duration.
