Isolated femoral vein obstruction may become the cause of development of severe post-thrombotic syndrome (PTS) deteriorating the patient’s quality of life. In certain cases, reconstructive surgery is the only effective method for the management of this condition. Objective of the study. To evaluate the effectiveness of saphenopopliteal anastomosis for the treatment of femoral vein obstruction. Material and methods. A total of 12 patients at the age from 41 to 69 years presenting with PTS caused by femoral vein obstruction were given the surgical treatment. All of them suffered from oedema and seizures refractory to conservative therapy, feeling of heaviness in the affected lower extremities, venous claudication, and hyperpygmentaton (G4S). Six patients had concomitant lipodermatosclerosis (G4bS). The patients were examined by ultrasound angioscaning and phlebography for the evaluation of the changes in superficial and deep veins. All patients underwent saphenopopliteal bypass surgery within 2—5 years after the onset of femoral vein thrombosis with the transposition of the great saphenous vein (GSV) into a popliteal vein. In 4 patients, the extravasal GSV support was formed using the Vedensky frame spiral. Results. The long-term results of the treatment were monitored during a follow up period of 60±11 months in ten of the 12 treated patients. In all of them, the shunt remained patent throughout the entire period of observations. The disappearance of convulsions, venous claudication in the affected limb, and the decrease of the hyperpigmentation area were documented in the ten patients. Oedema was totally eliminated in 3 patients while 8 patients reported the absence of heavy feeling in the legs. The relief of clinical symptoms of PTS in the form of regression of leg oedema was documented in 7 patients, 2 others experienced less pronounced feeling of heaviness in the affected legs after surgery. Recanalization of the femoral vein with the development of axial reflux developed in 2 patients within 1 year after surgery. Conclusion. Saphenopopliteal anastomosis is the efficacious method for the treatment of post-thrombotic syndrome in the patients with isolated femoral vein obstruction.
