One of the causes of portal vein thrombosis and the formation of extrahepatic portal hypertension may be previous operations on the organs of the hepatoduodenal zone. Two cases of successful treatment of patients with extrahepatic portal hypertension, multiple bleeding into the lumen of the gastrointestinal tract (in the area of hepaticoejunoanastomosis, imposed earlier) are presented. In both cases, cavernous transformation of the portal vein was formed as a result of its thrombosis caused by the inflammatory process against the background of the course of the underlying disease and the postoperative period, as well as varicose veins of the hepatoduodenal ligament. Patients underwent surgery — laparotomy, mesentericocaval anastomosis «side to side». The immediate postoperative period is smooth. Under dynamic observation, bleeding into the lumen of the gastrointestinal tract did not recur. The observation period in one case is two years, in the second — one year.
