Objective — to compare the efficacy of the medication regimen of mifepristone and misoprostol for terminating an undeveloped pregnancy in the second trimester, and the use of the same regimen with the addition of cervical preparation by intracervical administration of an osmotic dilator. Material and methods. 74 patients with antenatal fetal death in the second trimester were examined. In the 1st group (n=37), in addition to the pregnancy terminating drug regimen (mifepristone 200 mg, misoprostol 800 µg), a hygroscopic osmotic dilator was intracervically administered. In Group 2 (n=37), only the drug regimen for abortion was used (mifepristone 200 mg; misoprostol 800 µg). Results. In the 1st group the miscarriage occurred 2 times faster (p=0.0000) than in the 2nd group. The volume of blood loss in the groups did not differ (p=0.5741). In the 1st group, 45.9% (17) of patients had complications from hematometra in the post abortion period. In the 2nd group, complications were found in 59.5% (22) of the patients, among whom 21 (95.5%) had hematometra; and 0.5% of the patients had tissue remnants of the fetus in the uterine cavity. Statistical processing showed that the risk of post abortion complications is the same in both groups — (odds ratio 0.580 (0.231;1.456), p>0.05. Conclusion. The simultaneous use of an osmotic dilator and mifepristone and misoprostol to terminate an undeveloped pregnancy in the second trimester reduces the time of miscarriage by 1.98 times, but does not reduce (p=0.2501) the probability of complications in the post abortion period.
