Objective — to develop a surgical method for strengthening the uterus in patients with severe apical prolapse and to evaluate the safety and efficiency of the method. Subject and methods. Sixteen patients aged 27 to 46 years with uterine prolapse were examined and operated on. The patients underwent transabdominal extraperitoneal strengthening of the uterus with synthetic flaps (Stage 1). The effect of surgery was evaluated before Stage 2 (colpo-peritoneal levator plasty). Results. Following surgery, all the patients were found to have anterior and/or posterior vaginal wall prolapse with a good apical fixation. The severity of anterior vaginal wall descent could be eliminated or reduced to subclinical forms in almost all cases. Most patients underwent second-stage surgery, such as colpo-peritoneal levator plasty. Moreover, there was a decrease in the number of patients with complaints of stress urinary incontinence. Conclusion. This technique has shown the high efficiency of treatment and the low frequency of complications and may be used to treat women with severe apical prolapse who wish to preserve reproductive function.
