Objective. To study the significance of video laparoscopic technologies in the diagnosis and treatment of early intra-abdominal complications after gynecological surgery. Subject and methods. The results of treatment were analyzed in 3440 patients who had undergone abdominal interventions for different gynecological diseases, of whom 122 (3.54%) developed postoperative complications. There were two clinical patient groups: 1) 62 patients with early postoperative complications who had undergone video endoscopic interventions (a study group); 2) 60 patients with the same complications who had undergone traditional open laparotomies (a control group). Results. All postoperative abdominal complications (postoperative peritonitis, abdominal abscesses, early ileus, and intrauterine bleeding) required reoperation in 42 patients. At the same time, only laparoscopy was a final treatment in all 20 patients of the study group. In the control group, complications were treated via a minimal laparotomic approach in 4 patients; diagnostic laparoscopy established that 10 patients in the same group had indications for relaparotomy. The latter was carried out immediately in only 8 control patients with abdominal complications. Conclusion. In suspected early postoperative intra-abdominal complications after gynecological surgery, video laparoscopic technologies are an alternative to diagnostic relaparotomy.
