The efficiency of different delivery options was studied in 60 women with advanced pregnancy of 41-42 weeks of more of gestation. The women's age ranged 18 to 35 years. The fetoplacental unit was examined by ultrasound and Doppler studies. The maturity of the cervix uteri was rated using the scale developed by E. Bishop. A control group included 30 women at 39-40 weeks gestational age, in whom the labor started spontaneously and ended in a vaginal birth. To prepare the cervix uteri for delivery, prostaglandin E2-containing gel was intracervically administered to 52 women with advanced pregnancy, by keeping in mind the changes occurring in maternal-placental-fetal blood flow. The use of the prostaglandin gel was ineffective in 4 and 16 women from Groups 2 and 3, respectively, which caused cesarean delivery to be performed. Based on the resultant good perinatal outcomes of delayed labor, the authors recommend that one should comprehensively investigate the fetoplacental system, to determine the readiness of the woman's body for birth, to choose a delivery option, and to perform the latter as soon as possible.
