Abstract:

Controlled ovulation induction was made in 48 patients with anovulatory infertility, caused by hypothalamic-pituitary dysfunction, in the first attempt using clomiphene citrate; in the early detection of signs of clomiphene resistance, recombinant follicle-stimulating hormone (rFSH) was also administered in this cycle; if there was no effect, the step-up protocol using the low starting doses of rFSH was applied in the second and third attempts. Only the traditional ovulation induction protocol using menotropins was employed in 12 patients with hypothalamic-pituitary insufficiency. The paper presents the new data on the therapeutic efficiency, safety, and economic expediency of the sequential use of a combination of rFSH with clomiphene citrate and the low-dose step-up protocol with rFSH, which let one consider these two regimens as a justifiable alternative to the traditional gonadotropin dose protocol used in clomiphene-resistant patients in repeated ovulation induction cycles.
