OBJECTIVES
Develop a scale to measure implementation of Federal Tobacco Control Law “on protecting the public health form tobacco smoke and consequences of tobacco consumption” and evaluate the implementation of tobacco control legislative measures in 10 Russian regions.
MATERIAL AND METHODS
Cross-sectional data from Russian tobacco control policy evaluation survey EPOCHA-RF are analysed — a representative survey, based on multistage territorial sampling in 10 Russian Federal subjects in 2017—2018, stratified by smoking status: n=11625, including: 6569 smokers, 2377 former smokers, 2679 never smokers.
The Tobacco Control Law (TCL) Implementation scale is developed, based (1) on implementation of tobacco control law articles, including World Bank six cost effective tobacco control measures in 10 Russian regions and (2) scoring system, developed by international panel of tobacco control experts (L Joossens, M Raw 2006.15(3)). Implementation of Tobacco control law was compared in 10 Russian Federal subjects, the score measured by the formula: = X /Xmax·Wx, where is the indicator of the TCL measure, Xmax — its maximum value in the column and Wx — the weight of the measure, according international scoring system.
RESULTS
The overall TCL implementation score ranged from 93.1 (out of 100 possible) in the Chuvash Republic to 65.9 in the Tyumen Region, and averaged 73.9 in 10 regions. For implementation of effective Price policy, the scores ranged from 28.9 (out of 30) in the Chuvash Republic to 9.6 in the Tyumen Region, (17 on average); Smoke-free policies — from 21.5 (out of 22) in the Belgorod Region to 17.8 in the Krasnodar Territory (on average 19.6); Information communication policy: from 14.3 (out of 15) in the Orenburg Region to 11 in the Novosibirsk Region (on average 12.7); banning TAPS policy from 13 (out of 13) in the Chuvash Republic to 9.7 in the Belgorod Region (on average of 11.2); Health warnings perceptions — from 8.4 (out of 10) in the Krasnodar Territory to 6.4 in the Chuvash Republic (an average of 7.7); Smoking cessation support — from 8.8 (out of 10) in the Chuvash Republic to 3.4 in the Primorsky Territory (an average of 5.2).
CONCLUSIONS
The developed integral TCL Implementation scale allows to measure the implementation of Federal tobacco control legislation in Russian regions, conduct interregional comparisons, monitor the overall performance, and also implementation of separate tobacco control measures, take action for better law enforcement.
