Objective — to evaluate the effect of different surgical hemostasis on ovarian reserve in women of reproductive age with benign ovarian tumors and cysts treated with urgent operations. Material and method. Histological examination of the removed ovarian tissue from 163 patients was performed. Ovarian reserve was assessed in 83 patients after laparoscopic resection of the ovary. Three groups were formed depending on the method of hemostasis: group I (n=30) with the use of suture material, group II (n=25) with electrocoagulation, group III (n=28) with single ovary, suture and electrocoagulation. Results. Histological examination of the removed ovarian tissue showed that monopolar coagulation lead to necrosis of the cortical zone of the ovary, penetrating the full thickness of the removed tissue in 21 (70.0%) of cases. The depth of bipolar coagulation was less than 25—30% in all samples. A significant decrease of AMH after 3 and 6 months after surgery in patients of group II compared to group I and patients of group III compared to I and II was revealed. Conclusions. Hemostasis with suture material is less traumatic for ovarian reserve. Women after surgery on the ovary are at risk for premature ovarian insufficiency.
