Abstract:

OBJECTIVE

To increase the effectiveness of treatment of micrognathia in children with Goldenhar syndrome using preliminary surgical preparation including autologous fat injection into facial soft tissues in projection of further lower jaw distraction.

MATERIAL AND METHODS

The study included 29 children with Goldenhar syndrome, who underwent preliminary surgical preparation before compression-distraction osteosynthesis (CDO) (the main group). The control group included 25 children without PSP before CDO. In the main group, we analyzed preoperative manifestations of congenital paresis using the House-Brackmann scale.

RESULTS

In the main group, baseline paresis persisted after fat injection and CDO in all patients. In the control group, paresis increased in 9 (36%) children after CDO, and we recognized this result as an unsatisfactory one.

CONCLUSION

Preliminary autologous fat injection into soft tissues in the area of further distraction prepares all tissue layers for distraction and minimizes soft tissue deformation after correction of mandibular deformity.
