Abstract:

BACKGROUND

To study the microsurgical anatomy of the interpeduncular cistern and its arachnoid membranes.

MATERIALS AND METHODS

The interpeduncular cistern and the arachnoid membranes were studied on 5 fresh cadaver preparations and 14 fixed cadaver head-neck complexes with colored silicone vessels.

RESULTS

The anatomy of the interpeduncular cistern and associated arachnoid membranes was studied. The steps of neuro-endoscopic operations in the area of third ventricle bottom were described. It should be emphasized that during surgical access the arachnoid membranes and trabeculae complicate the exposure of affected areas of the interpeduncular cistern. It becomes possible to expose adequately the structures of interpeduncular cistern only with accurate dissection of the BA bifurcation membrane and posterior perforated membrane after opening the diencephalic leaf of the Lilquist membrane during the intervention. Dissection of the diencephalic leaf and Liliquist membrane to create an opening in the interpeduncular cistern not only helps to reduce the brain retraction but also prevents the occurrence of postoperative hydrocephalus.

CONCLUSION

Understanding the microanatomy of the arachnoid membranes of subarachnoid cisterns is vital in neurosurgery, especially in minimally invasive neurosurgery. In particular, knowledge of the structures of the interpeduncular cistern and arachnoid membranes is necessary in endoscopic surgery of the third ventricle bottom.
