Abstract:

OBJECTIVE OF THE STUDY

To evaluate the short-term results of definitive treatment of cancer of the right half of the colon complicated by intestinal obstruction in a general surgical hospital.

MATERIALS AND METHODS

The treatment results of 157 patients after emergency surgical interventions for complicated colorectal cancer in a general surgical hospital in Moscow over 5 years were analyzed. A prospective study was conducted with a retrospective control group: at the retrospective stage, traditional surgical interventions were performed; at the prospective stage, mesocolonectomy and extended lymph node dissection (main group) were used. Postoperative complications were assessed using the Clavien-Dindo classification.

RESULTS

The frequency of intra- and postoperative complications in patients of the retrospective and main groups did not differ statistically (p>0.05). Despite the greater number of extensive operations performed in the main group, the incidence of infectious complications and mortality rates in the main group were similar to those in the retrospective group (p>0.05).

CONCLUSION

Implementing the developed algorithm for treating patients with cancer of the right half of the colon complicated by acute intestinal obstruction improved the quality of preoperative preparation, increased the number of extended definitive interventions in emergency patients, and reduced postoperative hospital stay.
