Abstract:

OBJECTIVE

To investigate the clinical features of malignant mediastinal neurogenic tumors and to evaluate the efficiency of their surgical treatment.

Subjects and methods. Data on 13 patients with malignant neurogenic mediastinal tumors who had received surgical treatment in the period from 2001 to 2020 were retrospectively analyzed. To determine their clinical features, benign neurogenic tumors (n=102) were comparatively analyzed. The immediate and long-term results of treatment were studied.

RESULTS

In the pattern of neurogenic mediastinal tumors, malignant neoplasms accounted for 11.3% (13/115). Compared to benign tumors, malignant tumors were characterized by large sizes (p<0.001), the high incidence of pain (p<0.001), and weakness (p=0.002) at the time of detection. Histological examination revealed a malignant peripheral nerve sheath tumor in 84.6% (11/13) and a primitive neuroectodermal tumor (PNET) in 15.4% (2/13). All the patients underwent radical tumor resections (R0). The median follow-up was 46.8 months. Local relapse was observed in 30.1% (4/13) of patients. The 5-year cancer-specific survival rate was 71.8%.

CONCLUSION

Surgical treatment for malignant mediastinal neurogenic tumors can yield long-term satisfactory results, but it is characterized by a high local recurrence rate; in this connection, it is necessary to study whether it is feasible to perform additional radiation or drug treatment using up-to-date technologies to improve long-term outcomes.
