Abstract:

The management of locally advanced and prognostically unfavorable rectal cancer involves combination treatment using chemoradiotherapy and surgery. The treatments based on the principles of drug modification of radiosensitivity have supplanted classical radiotherapy regimens. Chemoradiotherapy versus radiotherapy has demonstrated better results in the basic criteria: the magnitude of tumor regression, the rate of local recurrences, the functional and long-term results of treatment. Neoadjuvant chemoradiotherapy is indicated for patients with rectal cancer who cannot undergo R0-resection according to preoperative examination data and for those with Stages II and III (except for cT3a,b stages) and at the probabilistic margin of circular resection less than 1 mm with the tumor being located in the lower and middle ampular portion of the rectum.
