Mitochondrial genetics and function play a central role in the development, course and outcome of multiple diseases across all organ systems. Nonetheless, the assessment of mitochondrial function in clinical medicine is difficult, primarily due to the invasive procedure in order to get a viable tissue sample for bioenergetic analyses and the lack of adapted laboratory technologies. In recent studies, a hypothesis has been formulated according to which the mitochondria of easily accessible circulating blood cells can be used as a biomarker for systemic and/or cell-specific bioenergetic function. The present review will highlight the technologies for assessing the bioenergetics of blood cells and clinical studies in which these assays have been used. Data on blood cells bioenergetic status in healthy individuals and various pathologic conditions with regard to clinical parameters are summarized. We also discuss the challenges and ways for implementation of bioenergetic assays in the field of translational research and personalized medicine.
