Melanocytic nevi in children are found on the entire skin. By the end of the first decade of life in children of I–IV photo-types, on average, 15 to 30 nevi are observed, in children of V–VI photo-types — from 5 to 10 nevi. Although melanoma is extremely rare in childhood, parents often express concern about re-emerging nevi in children or the growth of existing ones, leading to unnecessary aggressive interventions. The excessive surgical activity of doctors, on the one hand, is explained by the deep conviction of doctors that it is necessary to remove melanocytic formations in order to prevent melanoma, on the other hand, the anxiety of parents and the firm belief that a child can have nevi that can transform into melanoma. The decision on the tactics of conducting nevi in a child should be determined by reasonable data presented by the scientific community in modern literature.
The analysis of literature data over the past 10 years on the pathogenesis, clinical features, diagnosis and management of congenital and acquired nevi in children was the goal of this work. The article provides up-to-date information on the clinical, dermatoscopic and morphological features of congenital nevi, Spitz/Reed nevi and nevi of special localizations. This is a group of melanocytic neoplasms that cause the greatest alertness among doctors and concern among parents. Based on the data from the world literature, we propose the tactics for managing nevi in children, including the analysis of the clinical picture, dermatoscopic images and prognosis of the course.
