The article discusses detection of eosinophils in patients with autoimmune pemphigus. The presence of eosinophils in the contents of subepidermal bullas and peripheral blood is considered to be a characteristic feature of dermatitis herpetiformis (Duhring's disease). In bullous pemphigoid, macrophages and eosinophils first accumulate at the basal membrane, then migrate through the latter and begin to accumulate within the bulla and between the basal cells. In autoimmune pemphigus, dermal infiltrate contains eosinophils along with plasmacytes and lymphocytes. However, in typical cases, there are no eosinophils in the contents of intraepidermal bullas, resulting from acantholysis. We report two clinical cases of pemphigus vulgaris, when 27—52% eosinophils were detected in bulla contents in female patients aged 47 and 63 years. The patients had bullous elements sized 3 to 5 cm in diameter on the face, scalp, trunk, and limbs; mucous membranes of the mouth and vermilion zone had erosive defects up to 1—2 cm in diameter; positive Nikolsky’s and Asboe—Hansen’s signs were observed. Acantholytic cells were found. Histological examination revealed intraepidermal bullas and the areas of suprabasal acantholysis.
