OBJECTIVE

To evaluate various methods of surgical revascularization of deep femoral artery in patients with occlusion of autologous venous femoral-popliteal bypass grafts.

MATERIAL AND METHODS

We analyzed treatment outcomes in 34 patients with occlusion of autologous venous femoral-popliteal bypass grafts. Occlusion-free period was 45.3±12.0 months. All patients were divided into 2 groups depending on revascularization technique: 20 patients underwent endarterectomy from common and deep femoral arteries (group 1), 14 patients underwent balloon angioplasty with stenting of deep femoral artery (group 2). There were no significant between-group differences in comorbidities.

RESULTS

Surgical site infection occurred in 1 (2.9%) case. Healing of trophic disorders within 30 days after surgical treatment was observed in 7 out of 14 patients (50%). One patient underwent redo surgery. Two patients underwent foot amputation within 12 postoperative months.

CONCLUSION

Deep femoral artery revascularization has positive results in patients with chronic lower limb ischemia following occlusion of autologous venous femoral-popliteal bypass grafts. However, blood supply to the lower extremity through the deep femoral artery may be insufficient for healing of trophic ulcers.
