Abstract:

OBJECTIVE

To compare clinical effectiveness and safety of standard and modified supracoronary myotomy in postoperative period.

MATERIAL AND METHODS

A retrospective analysis included 47 patients after off-pump supracoronary myotomy (SM). Standard procedure was carried out in 20 patients, modified approach — in 27 cases. This modification implies creation of sufficient diastase between the edges of excised muscle above the coronary artery. This method is recommended for prevention of adhesions over the decompressed coronary artery and, as a result, recurrence of angina in long-term postoperative period.

RESULTS

Mean length of myocardial bridge was 4.7±3.2 cm, depth of occurrence — 3.1±1.7 mm. Early postoperative period was uneventful. After 6 months, all patients were re-examined and completed a survey using the SAQ questionnaire. According to stress echocardiography data, there were no signs of myocardial ischemia, and control coronary angiography did not reveal external compression or other signs of myocardial bridge. Survey showed a significant improvement in the quality of life in both study groups.

CONCLUSION

A 6-month postoperative follow-up did not reveal the advantages or disadvantages of the proposed modification of supracoronary myotomy in comparison with standard surgery. The outcomes were satisfactory in both groups. Our study is an interim one. Further follow-up of patients is required to evaluate long-term postoperative results.
