Objective. To analyze in-hospital results of femoropopliteal bypass/replacement using diepoxy-treated biological prosthesis. Material and methods. There were 259 procedures of femoropopliteal replacement and 56 bypass surgeries in patients with atherosclerotic lesion of the lower limb arteries for the period 2002—2016. Diepoxy-treated biological prosthesis was used as a conduit. Inclusion criteria were indications for femoropopliteal bypass/replacement and no need for cerebral and myocardial revascularization. Exclusion criteria were critical lower limb ischemia, severe concomitant diseases limiting postoperative follow-up. Results. The most common postoperative complication was lymphorrhea (n=21, 6.7%). Anastomotic leakage due to eruption of the ligature occurred in 4 (1.1%) patients that required additional sutures. One (0.3%) patient died from hypertensive crisis complicated by hemorrhagic stroke. Early prosthesis thrombosis developed in 10 (3.2%) patients that required urgent redo revascularization (replacement — 2 (0.6%) cases, thrombectomy — 3 (1.0%) cases, prosthetic-autovenous bypass below the knee-joint fissure — 5 (1.6%) patients). Thrombosis was caused by detachment of intima within anastomotic zone in 5 (1.6%) cases, underestimation of peripheral circulation (popliteal artery stenosis) — in 5 (1.6%) patients. Conclusion. Diepoxy-treated biological prosthesis is safe and effective for reconstruction of lower limb arteries. The most common postoperative complications are conduit thrombosis and lymphorrhea. However, incidence of these events is quite low (less than 7%).
