Abstract:

Abstract Objective — to determine the optimal material and length of the patch for profundoplasty in patients with critical lower limb ischemia. Material and methods. There were 76 patients who underwent surgery for critical lower limb ischemia. The examination was conducted in accordance with the TASC II guidelines: Doppler ultrasound with registration of ankle arterial pressure and ankle-brachial index, contrast CT-angiography. Results. ABI after 5 years was higher in patients with extended profundoplasty (0.8) compared with conventional profundoplasty (0.66; p<0.05). Patency of reconstruction area after 5 years was higher after profundoplasty from autologous material (79.4%) compared with synthetic materials (65.4; p<0.05). Five-year freedom from amputation was higher after profundoplasty from autologous material (82.4%) compared with synthetic materials (67.5; p<0.05). Conclusion. Extended profundoplasty from autologous material is preferred in patients with critical lower limb ischemia and compromised distal vessels.
