Aim — to analyze efficacy of internal carotid artery stenting in advanced age patients. Material and methods. There were 66 patients who underwent internal carotid artery stenting (71 stents) in the Inozemtsev Municipal Clinical Hospital from January 2014 until now. Mean age was 74.4±2.2 years. Ultrasound of brachiocephalic arteries, contrast-enhanced computed tomography of intra- and extracranial arteries and neurological examination were applied prior to surgery in all cases. Results. Following stents were deployed: Xact Carotid Stent System — 5 (3,6%) patients; Acculink RX — 2 (1,4%), Cristallo Ideale — 41 (29,1%), PRECISE PRO RX — 23 (16,3%). Distal cerebral protection devices were used to prevent material embolism: Accunet RX — 12 (8,5%) patients, Emboshield NAV6 — 20 (14,2%), AngioGuard RX — 31 (22,1%). Critical ICA stenosis was managed using Mo.Ma Ultra proximal protection system in 14 (9,9%) patients. Mean time of carotid arteries occlusion was 8±2 min. Stent deployment was followed by post-dilatation using balloons 5—6 mm. Procedures were technically successful in all patients. Mortality was absent. Conclusion. In our experience, endovascular approach for carotid stenosis is minimally invasive and effective method in advanced age patients who have severe comorbidities and high risk of open surgery.
