Abstract:

Aim — to analyze immediate and long-term results of ULAE management. Material and methods. Experience of 1147 ULAE patients management within 47 years is presented. Long-term outcomes were assessed in 315 cases in terms of 1—25 years. Statistical processing was made with SPSS 17.0 (SPSS, Inc., Chicago, IL, USA). Results. ULAE was predominantly observed in women. Age over 60 years was in more than 80% of them. Cardiac embolism (65.1%) due to atrial fibrillation was predominant. The most frequent embolus fixation point was brachial artery (60.9%), less often — axillary (21.4%) and subclavian arteries (13.8%). Ischemia grade I and IIA was revealed in 55.8 and 29.8% respectively. Irreversible limb ischemia was in 0.8% of cases. 90.8% of patients underwent urgent surgery, blood circulation in the extremities was restored in 97.2% of patients, overall mortality was 9.7%. Overall 1, 5 and 10-year survival was 69.4±5.2, 28.2±5.6 and 12.6 ±5,4%, respectively. Conclusion. High early postoperative mortality and low long-term survival are associated with course of the main (embologenic) disease and its complications, that requires their timely correction and prevention of recurrent thromboembolic events.
