Abstract:

Аim. Of the study was to Conduct a comparative analysis evaluating the efficacy of reconstructive operations and conservative treatment in patients presenting with pathological tortuosity of the internal carotid artery. Material and methods. The study included 63 patients. Diagnostic algorithm: a study of hemodynamics of the internal carotid arteries, evaluation of neurological status and methods of examining the eye. Patients were divided into two groups: group one patients (37) and conservative treatment (26). The neurological status of patients was as follows: asymptomatic — in 25 (39.7%), transient ischemic attack 18 (28.6%), encephalopathy in 12 (19%), stroke in 8 (12.7%) patients. Ophthalmological symptomatology was predominantly manifested by attacks of amaurosis fugax — in 19 (30.2%) patients and processes related macular degeneration — in 49 (77.8%) patients. Results. We assessed the immediate (30 day) and long-term (1 year) results of conservative and surgical treatment. In group I, marked the cessation of attacks of amaurosis fugax (32.4%), improvement of visual acuity of 0.1 or more (29.7%) (p=0.02), expanding the boundaries of fields of view (35.1%), disappearance of cattle (24.3 per cent) (p=0.01) and stop processes related macular degeneration (56.8%) (p=0.04) (after 1 year). Patients with transient ischemic attacks and dyscirculatory encephalopathy moved into the category of asymptomatic patients (75.7%) (p<0.05). In group II, episodes of amaurosis fugax disappeared only in 2 (7.7%) patients (p=0.02) and neurological symptoms were less severe and the category of asymptomatic amounted to 46.2% (p<0.05). Conclusion. Surgical treatment has greatly improved as neurological and ophthalmological status, and efficacy of reconstructive operations was more pronounced in patients with acute forms of GIS.
