Aim. To present the successful experience of temporary intraluminal shunting and algorithm for shunt’s safe installation in brachiocephalic arteries reconstruction. All operations were performed in the department of cardiac surgery of I.M. Sechenov First Moscow State Medical University for the period 2009—2014. Material and methods. 70 brachiocephalic reconstructions in 66 patients were performed. Temporary intraluminal shunting was applied in 15 patients. A detailed protocol for the use of temporary intraluminal shunt and safe algorithm for its installation during reconstructive surgery are expounded. Results. Use of described technique allowed achieving good immediate results of reconstructive surgery in 65 patients. There were no any complications associated with temporary intraluminal shunting and deaths. Postoperative ipsilateral stroke was observed in 1 (1.5%) patient. Temporary intraluminal shunt was not applied in this case.
