Aim — the objective if the present study was to estimate the effectiveness of the prevention of recurrent trophic venous ulcers with the use of special knee-high compression stockings. Material and methods. The study included 20 patients at  the age of 40—70 years presenting with the healed trophic ulcers (C5 according to CEAP). Wearing special knee-high compression stockings was prescribed to prevent the  relapse of ulceration during 12 months. The main characteristics recorded throughout the study period included the clinical manifestations of chronic venous insufficiency (CVI) and the changes in the microcirculation system. Results. The pressure of the knee-high stockings (around 40 mm Hg) persisted throughout the observation period. The clinical manifestations of chronic venous insufficiency, such as oedema, skin induration and hyperemia significantly decreased by the end of the study. Also, the changes became apparent that gave evidence of the improvement of microcirculation in the form of a reduction in the number of 2—3 type capillaries and the appearance of «classical» unaltered type1 capillaries. At the same time, the capillary flow velocity increased while the ratio of their diameters changed with the predominance of the  transition segments, and the perivascular zone. Conclusion. The adequate compression treatment has beneficial influence on the regression of clinical symptoms of chronic venous insufficiency in the lower extremities and improves the parameters of microcirculation. These effects confirm the necessity of the regular application of elastic compression of the lower extremities. Daily wearing knee-high compression stockings is convenient, does not require special instructions, and can be recommended for the effective prophylaxis of  recurrent trophic venous ulcers.
