Aim. The objective of the present study was to estimate the clinical and economic effectiveness of the long-term local application of negative pressure (technical vacuum) using special dressings for the treatment of venous trophic ulcers. Material and methods. This prospective comparative non-randomized study included 21 patients presenting with class 6 chronic venous disease allocated to two groups. In one of them, the patients were treated with traditional topical preparations chosen depending on the phase of the wound process and species specificity of the causative pathogen. Nine patients of the second group underwent negative pressure wound therapy. Results and discussion. The two groups were not significantly different in terms of the age and sex of the patients, nosological variant of the chronic venous disease, the size and microbiological characteristics of the ulcers. The mean duration of hospital stay prior to autodermoplasty was reduced from 15.7±0.3 to 9.6±1.8 days (p=0.004) and the mean total length of the hospitalization period from 22.8±3.8 to 15.1±1.9 days (p=0.043). The average cost of the treatment of one patient decreased from 69,433.52±11,552.8 rubles to 53,332.09±5,968.4 rubles (p=0.117). No recurrent ulcers were documented in the group of patients who underwent local application of negative pressure. Conclusion. The local application of negative pressure for the combined treatment of the patients with venous trophic ulcers is a highly promising method for the management of this condition that allows all aspects of therapy to be significantly approved.
