The present work had the objective to study the state of the major veins in the proximal segment of the left thigh in patients with varicocele. Six of the 182 patients presented with the subclinical form of this pathology, 18, 108, and 50 ones had grade 1, II, and III varicocele respectively. The age of the patients ranged between 14 and 49 years (mean 17.9 +- 2.2) years. It was shown that lesions of the venous wall were not confined to the scrotal vessels alone; they also extended to the left femoral veins that responded to physical exercises by luminal narrowing. This reaction was more pronounced in the evening than in the morning, immediately after the night-time rest in the horizontal position. Visualization of 51 (28.0%) patients by duplex scanning revealed left-side varicocele and communicating vessels originating from the veins of the spermatic cord and opening directly into the veins of the proximal segment of the left thigh. Iliospermatic reflux occurred through these vessels. Three variants of the anastomoses of these vessels with major veins are distinguished: some of the open directly into the common femoral vein, others in the sapheno-femoral junction and the great saphenous vein.
