Results of the treatment of primary varicosis of the lower extremities were compared in three groups of patients presenting with thrombi in the great saphenous vein (GSV) system. Group 1 was comprised of 48 patients (48 extremities) treated by miniinvasive phlebectomy, group 2 included 56 patients (74 extremities) who underwent crossectomy in combination with foam echosclerotherapy of the saphenous trunk, group 3 consisted of 45 patients (51 extremities) who experienced radiofrequency obliteration (RFO). The immediate and mid-term outcomes of all treatments were analysed based on the global quality of life index (GQLI). Cost-effectiveness of each method was evaluated in terms of total expenditures. Ostial reflux was eliminated in the immediate postoperative period in 100% of the patients in groups 1 and 2 and 87.1% in group 3. The vertical reflux in the saphenous trunk was diagnosed only in patients of group 2 (7.2% and 30.0% within 12 and 36 months after surgery, respectively). GQLI was lowered after 12 months in all the patients in the absence of significant difference between the groups. The total treatement costs were estimated at 44,300 rubles in group 1, 30,080 in group 2, and 22,400 in group 3. It is concluded that miniinvasive phlebectomy and radiofrequency obliteration are the most efficacious methods for the treatment of primary varicosis in the saphenous vein system in terms of clinical outcome and cost effectiveness. The expenditures for the management of RFO are 1.9 times as low as for phlebectomy.
