Superior mesenteric artery syndrome (SMAS) is rarely established in the practice of abdominal surgery. The authors present a clinical case of the possibility of early diagnosis of this disease by using computer tomography (CT) with contrast and surgical correction of SMAS. The difficulties of diagnosing SMAS in clinical practice are due to the absence of specific symptoms. In establishing, the diagnosis of SMAS represents certain difficulties and is most often revealed when conducting additional research methods. One of the most promising directions in the diagnosis of SMAS is the use of CT of the abdominal bodies with contrast of the stomach and duodenum per os water-soluble contrast and computer angiography of vessels. This combination of methods allows you to estimate the functional state of the stomach and the duodenum, clearly visualize not only the topography of the organs but also evaluate the aorta-mesenteric angle and distance. With the aim of treating chronic duodenal obstruction, the decision was made to perform the most sparing and minimal surgical treatment Strong’s operation.
