A rare complication of endoscopic trans-papillary intervention is presented — trauma of liver tissue with a Glisson capsule with the formation of a hematoma, bleeding and bile leakage into the abdominal cavity; treatment of complications using laparotomy and postoperative endoscopic surgery. A 41-year-old patient with cholelithiasis, cholangiolithiasis complicated by obstructive jaundice on the 1st day after endoscopic retrograde cholangiography, endoscopic papillotomy, and stenting of hepaticocholedochus with a plastic stent was diagnosed with a rupture of liver tissue with bleeding and bile leakage into the abdominal cavity and hemoptysis. On urgent indications the laparotomy and tamponade of the liver hematoma cavity were performed. On the 1st day after laparotomy the bile leakage into the free abdominal cavity was noted; it was stopped by stenting and nasobiliary drainage of the right hepatic duct.
