The aims — comparative analysis direct surgical results of reconstruction techniques after a low anterior resection of the rectum. Material and methods. A retrospective analysis results of a prospective randomized controlled trial part of the rectum T1-4aN0-2M0. Results. In Group A was set 22 patients with J-shaped reservoirs, in group B with anastomosis side to end — 30 patients. Control group B with end-to-end anastomosis included 38 that is associated with intraoperative conversion of anastomosis due to anatomo-constitutional features of patients. Reason conversion: narrow male pelvis and visceral obesity, inadequate length of the descending colon. Significant differences in the incidence of postoperative complications between groups it was not revealed: 13.6% — in group A, 16.7% — in group B and 34.2% in the control group C (p=0.705). Using the minimally invasive total mesorectal excision surgeries have been significantly longer and averaged 230 min (110—380), with open TME median corresponded to 180 min (110—240) (p=0.001). Conclusion. Formation of J-shaped reservoirs in comparison with other types of anastomosis have more restrictions associated with male factor, visceral obesity and insufficient length the inferior parts of the colon. The immediate surgical results in the group with J-shaped reservoirs are comparable with the control and the main group (anastomosis side to end).
