Implementation of national projects and state programs of improvement of national health care resulted significant changes in emergency medical care system.
OBJECTIVE
To evaluate emergency medical care delivered by the specialized teams of anesthesiology and intensive care in ambulance and in-hospital departments in the Russian Federation in 2014-2019.
MATERIAL AND METHODS
Data on emergency medical care for 2014-2019 were analyzed using statistical, analytical, normative and law methods.
RESULTS
Analysis of emergency medical care delivered in different conditions showed a decrease in the number of emergency medical stations over the last 6 years and higher number of ambulance teams. The number of paramedic teams is increased in the structure of ambulance medical teams that is accompanied by less number of emergency physician and specialized physician teams. Advanced age patients prevail in the structure of calls including those accompanied by lethal outcomes. We found a decrease in the incidence of visits to patients with injuries, poisoning, sudden illnesses and conditions including road accidents with reduced number of deaths in an ambulance car. Analysis of in-hospital emergency medical care revealed the need for more than 3-fold scaling of the technology of inpatient emergency departments in the regions for effective emergency medical service in hospitals. Anesthesiology and intensive care teams account about 40% in the system of specialized emergency medical care. Scaling of the technology of inpatient emergency departments in the regions will require over 324 anesthesiologists to ensure round-the-clock work of intensive care units. Further improvement of emergency medical care predetermines a special role of anesthesiology and intensive care in availability and quality of medical care.
CONCLUSION
Analysis of ambulance service showed its positive development considering normalization of parameters of outpatient and in-hospital activity. Development of emergency and specialized medical care will be associated with interdisciplinary interaction of specialties with anesthesiology and intensive care.
