In modern conditions, a competent approach to the organization of orthopedic dental care involves special work related to the prevention and constructive resolution of conflicts between the patient and the doctor. To form a risk-based approach, it is necessary to analyze the main risk points during treatment and the nature of the consequences of the negative development of conflict situations.

OBJECTIVE

To determine the mechanisms of a risk-based approach in the organization of orthopedic dental care, taking into account the analysis of judicial practice for 2013—2022.

MATERIAL AND METHODS

The study analyzed the materials of medical reports of forensic medical examinations for the period 2013—2022 related to poor-quality dental orthopedic care.

RESULTS

The quality of cermet crowns and plate prostheses was most often evaluated during forensic medical examinations. Over the past 5 years, the proportion of court cases caused by poor-quality manufacturing of fixed and removable prostheses based on implants and structures made using CAD/CAM technologies has doubled. The main complaints were claims for improper bite restoration (in 49% of claims); unsatisfactory aesthetics of dentures (in 44%); inability to use dentures due to poor fixation (41%) or pain (54%); violation of technologies and structural modeling (in 59%). In 10% of court cases, payments from medical organizations amounted to more than 1 million rubles. On average, the amount of payments was 3.3 times higher than the cost of orthopedic dental treatment.
