Endodontic treatment of permanent teeth, in the case of chronic apical periodontitis that complicated by internal root resorption, seems to be highly relevant. A clinical case of root canal treatment of maxillary premolar with internal root resorption and wide apex was described in the article. In such clinical situation there are difficulties in proper root treatment as apical stop should be prepared to prevent gutta-percha extrusion during obturation.
