The article summarizes the factors and causes that affect differences in mortality rates between regions of the country and between countries. The main issues for comparison of mortality rates due to individual causes were determined: the concept of the prime cause of death (PCD) adopted 70 years ago does not meet the needs of modern health care systems and society as a whole; unclear clinical criteria for diagnosis of some diseases; inconsistency between ICD terms and clinical terms. The following are needed: revision of the PCD concept; clearer definition of the risk factor and cause of death; use of uniform international criteria for assessing the same event (condition/disease); development of derived classifications and their use in coding the causes of death; development of information systems for registration of all conditions that led to death; impersonal customized databases accessible for analysis; intelligent approach to the analysis of data entered by a physician when filling out a death certificate; and analysis of the coding errors probability.
