The authors have evaluated the appropriateness of the structure and contents of annual statistical Form No. 68, «Information on health center activities» in the regions of the Siberian Federal District in 2015 with the goals and objectives of health centers. There is redundancy of information in Tables 2001, 2002, and 2005 and its lack in Table 2009, as well as no need for it monthly submission. In addition, the Form lacks tables to collect information necessary to evaluate a number of important functions under legislation in health centers. It is concluded that there is a need to adjust the contents of Form No. 68 in accordance with the challenges facing the health centers and to elaborate instructions for filling out this reporting form with mandatory intra- and cross-table control.
