The efficiency of a physical rehabilitation and secondary prevention program in patients after prior acute myocardial infarction (MI) or endovascular intervention (EVV) into the coronary arteries (CA) depends on many factors and largely on their motivation and adherence to treatment. There are known special methods for their improvement, training according to the coronary heart disease (CHD) school program in particular. However, an analysis of the results of an investigation dealing with the rehabilitation of patients after EVV has shown that a combination of this training with a course of physical trainings causes a pronounced positive effect, including the participation of patients in long-term in-home trainings, which is possible only if they have a high motivation. Uncontrolled in-home trainings in the patients who have experienced acute coronary syndrome, including myocardial infarction, cause a good effect in enhancing motor activity and physical working capacity, on the one hand, and in reducing the number of cardiovascular events, on the other. The patients' adherence to physical trainings is likely to be extended over other treatment options used in CHD, which provides an explanation for the more favorable course of the disease in active versus inactive patients. In this connection, the uncontrolled (in-home) physical trainings must be considered as a basic (along with controlled trainings) nondrug method in the combined rehabilitation and secondary prevention programs for patients with CHD after MI and EVV into CA.
