ABSTRACT Female hypospadias (FH) is clinical manifestation of disorders of sex development, it is characterized several defects: shortening of urethra with ectopia of the meatus and vagina anterior wall defect. Elimination of the FH should be considered in conjunction with correction of other genital pathology. A huge number of surgical procedures were suggested including vaginoplasty, meatus transposition, introitoplasty, clitoris resection and labioplasty. Criteria of evaluation surgical correction is the female type of external genitalia (with separately opening urethra vagina), good urodynamic, absence of vaginal micturition, negative test O’Donnell—Hirschhorn. Chronic urinary-genital infections must be eliminated. Patient’s quality of life throughout the reproductive period because of optimal aesthetic and functional results of surgical correction should be high. Despite good results of surgical FH correction some complications such as recurrence of hypospadias, urethro-vaginal fistulas, stenosis of urethra and vagina, urethra diverticula and urinary incontinence still have place. Development of rational approaches to surgical correction FH, timing, evaluation of lasting results based on urodynamics, cosmetic indicators, individual characteristics of the external genitalia is an important medical and social challenge, which aims to improve the results of surgical correction of hypospadias and prolonged multidisciplinary treatment of girls with DSD.
