Objective. To estimate the factors influencing the choice, safety, tolerability and patient satisfaction of the use of an intrauterine system containing levonorgestrel (LNG-IUGS) and copper intrauterine device (Cu-IUD) within 1 year after installation in routine clinical practice. Material and Methods. The prospective cohort multicenter non-interventional comparative observational study included 2348 women aged 20—50 years. They used LNG-IUGS Mirena (n=1565) or Cu-IUD (n=778). Standard gynecological research was held, as well as the assessment of satisfaction by questionnaires and visual numeric scale. Results. The greatest influence on the choice of patients in favor of the LNG-IUGS had a beneficial effect on health (40%), of the Cu-IUD — economic component (40%). Use of LNG-IUGS for 1 year was accompanied by the decrease in frequency of dysfunctional uterine bleeding (DUB) in 3 times, menorrhagia — 5 times, dysmenorrhea — 10 times. Application Cu-IUD was associated with an increase in the frequency of menorrhagia in 15 times, dysmenorrhea — 3 times and DUB — 1.2 times. 97% of wearers of LNG-IUGS and 75% of users of Cu-IUD expressed satisfaction with the chosen method of contraception. Conclusions. The efficacy and safety of LNG-IUGS and Cu-IUD were comparable but the tolerability and satisfaction by an IUD was higher in LNG-IUGS users due to the positive influence on the menstrual cycle.
