Objective — to develop methods for fiber endolaryngeal surgery for laryngeal precancer and cancer. Subject and methods. In 1987 to 2018, endoscopic treatment has been performed in 260 patients divided into 5 groups: 1) 60 patients with dyskeratosis and Grade I—III dysplasia of the middle larynx; 2) 83 with recurrent laryngeal papillomatosis; 3) 65 with early cancer (Tis-T1N0M0) of the middle larynx; 4) 25 patients with residual cancer, including 20 after teleirradiation therapy (TIT) and 5 after endolaryngeal resection; 5) 27 with recurrent cancer of the middle larynx at 1—35 years after TIT. Results. Complete resorption was achieved in 96% of cases in Group 1, in 77.1% in Group 2, in 90.8% in Group 3, in 84.0% in Group 4, and in 81.6% in Group 5. The follow-up period ranged from 1 to 15 years. Complications were recorded in 3.8% of cases. Conclusion. The developed options of endolaryngeal surgery and photodynamic therapy using flexible endoscopic techniques are organ-preserving and functionally sparing treatments in patients with precancer and early cancer of the larynx.
