The paper describes the history of endoscopic techniques in the diagnosis of early central lung cancer. It presents current multimodal diagnostic methods, including confocal laser endomicroscopy and endocytoscopy, which make it possible not only to identify foci of intramucosal carcinoma in the tracheobronchial tree, but also to carry out life-time morphological examination at the cellular level. The long-term clinical experience gained by the P.A. Herzen Moscow Oncology Research Institute in diagnosing early central lung cancer, as well as pilot studies estimating the capabilities of confocal laser endomicroscopy and endocytoscopy in bronchial oncology are given.
