Abstract:

The paper describes a clinical case of long-term palliative treatment using pressurized intraperitoneal aerosol chemotherapy (PIPAC) in a gastric cancer patient with disseminated peritoneal carcinomatosis. Many specialized clinics, including the P.A. Herzen Moscow Oncology Research Institute (MORI), apply a multimodal approach encompassing cytoreductive surgery and hyperthermic intraoperative intraperitoneal chemotherapy (HIIC) to the treatment of peritoneal carcinomatosis. However, longer survival is noted mainly in a limited sample of patients with circumscribed dissemination (PCI <6—12) in case of complete cytoreduction (CC—0—1) or prophylactic HIIC in patients at high risk for peritoneal carcinomatosis. PIPAC is one of the innovative palliative treatments in patients with disseminated peritoneal carcinomatosis of various etiologies. Today the Thoracoabdominal Department, P.A. Herzen MORI, has performed 50 PIPAC sessions in 29 patients with peritoneal carcinomatosis from gastric cancer. Intraperitoneal aerosol chemotherapy in combination with systemic chemotherapy and endoscopic recanalization methods has demonstrated good results. This technique has established itself as promising, minimally invasive, and it may be used in cancer patients previously considered incurable.
