Abstract:

Objective: to assess the results of radical perineal prostatectomy (RPPE) in patients with low-risk progressive prostate cancer (PC). Material and methods. The data of 79 PC patients who had undergone surgery (RPPE) for localized or locally advanced PC at the Department of Urologic Oncology, P.A. Herzen Moscow Oncology Research Institute, were retrospectively analyzed. Results. Perineal prostatectomy (PE) versus retropubic (PE) failed to reveal significant differences in the detection rate of a positive surgical margin and in the risk of postoperative complications (p>0.05). The median follow-up was 50.5 (6—126) months. Postoperative biochemical relapse was recorded in 12 (15.2%) patients. In the total patient group, 5-year relapse-free survival (RFS) was 72.6±7.6%. That of RFS was 87±9.1% in the low-risk progression subgroup and 66.63±9.9 in the intermediate and high risk progression subgroup. In the total patient group, 5-year cancer-specific survival was 100%; overall survival was 94.1±0.9%. Conclusion. RPPE is an effective and safe surgical procedure in patients with PC. It may be recommended for patients with low-risk progression PC.
