Abstract:

The results of treatment were analyzed in 408 patients with large (3-cm or more) rectal adenomas. 158 (38.7%) patients underwent transanal endosurgery, 65 (15.9%) had transanal excision and 185 (45.4%) had endoscopic adenomectomy. Complications developed in 4 (2.5%) and 7 (10.8%) patients after transanal endosurgery and transanal excision, respectively (p=0.03). Four (2.2%) patients developed complications following endoscopic electroexcision. Tumor recurrences occurred in 11 (7.0%) patients after transanal endosurgery, in 13 (20.0%) after transanal excision, and in 33 (17.8%) after endoscopic electroexcision. Transanal endosurgery is significantly more effective than transanal excision (p=0.005) and endoscopic removal of large rectal adenomas (p=0.007). The size of a tumor and the pattern of its growth are factors that influence recurrence rates during all performed treatment options.
