The authors analyzed the results of anatomic hepatectomies using an ERBEJET2 water-jet dissector (ERBE, Germany). A traditional access was applied to 18 patients. A modified gated access with intraparenchymal excision of Glisson's bundles was used in 43 patients. The use of the water-jet dissector during anatomic hepatectomies for primary and metastatic tumors suggests that this procedure is safe and effective. The main benefit of water-jet dissection while applying the modified gated access to Glisson's pedicles is that precision detachment of the liver parenchyma from the hilar plate and exposition of Glisson's pedicle over the necessary length, without damaging the branches of hepatic veins, bile ducts are possible. On the other hand, the employment of the water-jet dissector when separating the parenchyma makes it possible to improve the visualization of vascular elements and to reduce the risk of traumatization of these structures, potentially causing a decrease in the volume of intraoperative blood loss and the frequency of postoperative complications.
