Abstract:

Lichen planus (LP) is an inflammatory disease that affects the skin, mucous membranes and skin appendages. About 25% of females suffering from the LP have vulvovaginal lesions. In 1982, M. Pelisse described one of the types of the erosive LP, which is characterized by a triad of symptoms, including vulvar, vaginal, and gingival lesions with pronounced tendency to scarring, adhesions, and stenosis. The diagnosis is established based on clinical examination and confirmed by histological examination of lesions. Vulvovaginal Gingival Syndrome (VGS) is characterized by chronic course, often relapses, and sometimes non-synchronous manifestation of certain symptoms. In the case of resistance to treatment and extensive process, systemic therapy with prednisolone, azathioprine, cyclosporine, griseofulvin, acitretin, synthetic antimalarials, metronidazole, and biological agents is carried out, which is not always effective and is often accompanied by adverse effects and complications. We have successfully applied the method of adaptive immunotherapy in patients with VGS, including photopheresis based on leukapheresis, UV-A irradiation of peripheral blood mononuclear cells isolated after administration of oral 8-methoxypsoralen followed by their subsequent reinfusion to the patient. We report a case demonstrating high efficacy of photopheresis in a VGS patient with the relapse-free period of more than 4 years.
