Arteriomesenteric compression of the duodenum comprises 1—2.5% of gastrointestinal diseases and results chronic duodenal obstruction in 3—17% of cases with mortality up to 33%. Despite various surgical interventions, there is still high incidence of various postoperative complications (6—15%) and mortality (5%). In this report, we present successful treatment of a patient with duodenal arteriomesenteric compression syndrome. The patient underwent infrarenal transposition of superior mesenteric artery. There were no postoperative complications. On the next day after surgery, the patient lost the taste of bile in the mouth. Solid food intake recovered in 3 days after surgery. Abdominal pain, nausea and vomiting disappear. CT angiography confirmed patent superior mesenteric artery and right angle of discharge from the aorta. The patient was discharged in 6 days after surgery. The next examination after 3 months revealed no complaints.
