Abstract:

Aim — to study clinical, hemodynamic and structural performance of small diameter bioprostheses in long-term postoperative period. Material and methods. Isolated aortic valve replacement with bioprostheses has been performed in 104 patients for the period 2007—2017. Mean age of patients was 69.8±6.0 years (54—88 years). Ninety-one (93%) out of 98 discharged patients were examined in long-term period. The following parameters were analyzed: peak and mean pressure gradients, interventricular septal thickness of the left ventricle (IVS), posterior wall thickness of the left ventricle (PWT), myocardial mass (MM), myocardial mass index (IMM), end-diastolic dimension of the LV (LV EDD), effective orifice area (EOA), indexed effective orifice area (iEOA). Results. Four (4%) patients died in the long-term postoperative period (mean follow-up was 5.9 years, range 9 months — 11 years). One (1%) redo surgery was performed due to infectious endocarditis. Dysfunction of the prosthesis due to structural degeneration was observed in 1 patient. Peak and mean pressure gradients were 25.4±6.9 (range 13—44) and 14.1±3.9 (range 7—25) mm Hg, respectively. Mean indexed effective orifice area was 0.9±0.1 (range 0.6—1.1) cm2/m2. Conclusion. Modern bioprostheses with an increased effective orifice area and anti-calcification covering provide good results of aortic valve replacement.
